I am lobbying for a similar idea in Minnesota.  I call it the LT fee method for life taken.  I like California's Demo as a kick-off but LT would add two more components; axle load and jurisdiction.
The fee would adjust to measure the life taken from the pavement and bridge structure and the cost of damage would be passed on to the jurisdiction owning the structure, i.e. city, county, towns and state.  We need to start an accounting method that measures the actual cost of moving goods and people, for far too long we have not assigned or tracked the total cost of embracing trucks and cars.

Thanks for offering copies of a typical invoice.  This system was one of a number of options suggested to our Wisconsin governor a couple years ago by his special trans. funding study group but then he totally ignored the report.  His mantra is no new taxes for transportation as our DOT faces a multi-billion dollar shortfall and projects are being delayed.
