Some time ago I had the privilege to participate at an ASCE Forensic Engineering Congress.
Based on what I heard experts in Forensic Engineering share, it occurred to me that
given every outcome (effect), e.g., bridge collapses, has an
"Upstream Driver" (cause).
It would be great to have a dialogue that answered that question about the
"Accident."
I have attached a paper I did right after that FE Conference to encourage such thinking.

"Engineered Accidents?"
