What I am observing, and it is happening more and more frequently, is that construction engineers are so busy with day-to-day bureaucracy and are failing to closely monitor the work in its details, such as checking the frame iron for lining before pouring the concrete.
This is what I have observed in works. And we engineers with more experience cannot let this happen.

While on site the civil engineer, here the question of discussion I am bringing forward is, what is the best way to managing failure on site construction in progress, perhaps this may happen. We are busy putting up a construction of highway bridge our effort is to avoid a future collapse, in other words , failures occur as a result of basic and established shortcomings inability to effect input of spatial layout of design that will allow a wider dissemination of key data analysis and calculations.
