And the loss of life from failed infrastructure a horrific way to then evaluate
"What went wrong?" *
By now I think we have learned that not routinely asking HDYK** of those
entrusted with signing off as to the safety of engineered projects is professionally
unacceptable.
What seems to concern some is the visibility external audits provide of
less-than-acceptable engineered works. And yet, that we assert
is part of our new COE:
"PREAMBLE . . .and above all else protect and advance the health, safety, and welfare of the public through the practice of Civil Engineering."
- Protect: i.e., safeguard, defend, shield, insulate, preventive, fail-safe
- Advance: i.e., achieve, accelerate, move up, propel, advocate, campaign.
Consider than the attachment with real-world engineered failures.
Clearly these failures were not caused by anything more complicated than
the lack of due-diligence in routinely having planned
"Phase-Gate QA" executed
per that project's plan, and reported out.
