     And at times significantly so?

    While the immediately popular response is assigned to technical issues, that is a "Downstream" observation.

    What has been reliably identified as early sources of poor performance are located within people, process, and leadership domains.

    Without specific identification of firms and projects, what issues, starting from deciding to propose to 'win' a project to project closeout do you identify as early candidates for continuous improvement?

    The attached paper offers related examples of often-overlooked "Early Candidates."
